Insurance Information

At Affiliated Podiatrists we will be glad to help you with your insurance needs and questions. Below you will find a list of the insurance plans in which our office participates. They include virtually every plan in the greater Milwaukee/southeast Wisconsin area. The only exception at the present time is the aurora direct plan. We do participate at a tier 2 level and we are working to become a tier 1 provider soon.

It is your responsibility to know if your insurance plan requires a referral. If it does, we need to have the written referral document in our office at the time of your visit. This usually takes 7-10 days to receive once you have requested it. Please allow enough time for us to receive the referral before your appointment time. If we do not have the referral when you appear for your appointment, we will call the insurance company to see if it is in their computer system. If so, we can still see you that day. If not, you will need to re-schedule your appointment for a later day unless you have a medical emergency.

We will file your insurance papers for you. If you have an office visit co-pay, we would appreciate it if you would pay it at the time of your visit. We accept visa and MasterCard in our office.
